This FORM must be used for all nominations submitted electronically. Complete the top section of the nomination form in its entirety. Select the appropriate category from the drop-down menu. Give as much information as possible to support your reasons for nominating this individual for this award. If you have not already done so, please read the Nomination Guidelines. A separate submission must be used to nominate a person in more than one category. Nominations must be received by April 1. Nominations may be submitted by mail using the downloadable Nomination form. Nominations received in any other format or after the deadline will not receive consideration by the Board.

PLEASE NOTE: Persons who have been Inducted into the Isshin-Ryu Hall of Fame are not eligible for other Awards.
